• 5% discount for payment in full at the time you schedule for treatment.

Care Credit™ is a convenient, low minimum monthly payment program that is accepted at our office and many other health services. Payment can be made with little or no interest. As our front office staff about Care Credit™ and the easy application process.

Dental Insurance

Though not a preferred provider of any dental plans we will assist with the completion of claim forms at the time of service. In order to process any claim properly we need to have all appropriate dental insurance company information which includes the following:

• Complete name of Dental Insurance Company
• Employer/Retirement Information
• Group Plan number
• Subscriber ID number and information (which includes name, date of birth, and social security number)
• Claim mailing information along with a phone number.

Any missing information prevents or slows the refund process. Your cooperation along with complete information at the initial visit speeds up the entire process.
